Utjecaj anteverzije femoralne komponente endoproteze kuka na primarnu stabilnost i periprostetički prijelom

Abstract

Produljenje životnog vijeka, ali i drugi faktori doprinose sve češćoj pojavi oštećenja zgloba kuka koje za posljedicu ima smanjenje pokretljivosti bolesnika. Ugradnja endoproteze zgloba kuka je kliničko rješenje za vraćanje funkcionalnosti zgloba i poboljšanje kvalitete života bolesnika. Ugradnja endoproteze podrazumijeva pravilnu orijentaciju acetabularne i femoralne komponente s ukupnim kutom anteverzije 25⁰-40⁰.
U radu je ispitana primarna stabilnost femoralne komponente s obzirom na kut anteverzije. U eksperimentalnom dijelu ispitivanja određeni su iznosi statičkog aksijalnog tlačnog opterećenja koja dovode do periprostetičkog prijeloma za tri skupine modela s endoprotezom implantiranom pod kutom anteverzije 0⁰-4⁰, 13⁰-17⁰, 26⁰-30⁰. Pomaci su tijekom ispitivanja mjereni optičkim mjernim sustavom Aramis. Za iste skupine modela provedene su računalne simulacije metodom konačnih elemenata.
Rezultati dobiveni eksperimentalnim i numeričkim ispitivanjem ukazuju na trend povećanja iznosa sile pri kojoj dolazi do loma uzorka s povećanjem kuta anteverzije. Iz rezultata računalnih simulacija vidljiv je značajan utjecaj manje vrijednosti kuta anteverzije na povećanje vrijednosti apsolutnog pomaka. Najveća kontaktna naprezanja između trupa endoproteze i kosti pojavljuju se na distalnom kraju trupa što potvrđuju mjesta lomova uzoraka kod eksperimentalnog ispitivanja.

Abstract (english)

The increasing lifetime expectancy, in combination with some other factors, causes hip diseases which result in lesser mobility and hip pain. Hip endoprosthesis is a clinical solution for regaining hip mobility and increasing the quality of life. Hip endoprosthesis implantation implies appropriate orientation of acetabular and femoral component with the total anteversion angle of 25⁰-40⁰.
In this paper the primary stability of femoral component, in regard to the anteversion angle, was tested. Axial static compression loads leading to periprosthetic fractures were determined in the experimental part of the testing. The test was run for the three sets of models in which endoprosthesis were placed with anteversion angles of 0⁰-4⁰, 13⁰-17⁰, 26⁰-30⁰. During the test the displacements were measured using the optical measuring system Aramis. Computer simulation using Finite Element Method was run for the same sets of models.
The results of the experimental and numerical tests indicate that the load value at which periprosthetic fracture of the sample occurs when the anteversion angle is increased tend to raise. The results of the computer simulations show the significant influence that lower degrees of anteversion have on total displacement value. Maximum contact stress between the endoprosthesis stem and the femur bone occurs at the distal part of the stem which corelates with the location of fractures determined in the experimental test.
