Abstract

Prijelom vrata bedrene kosti najčešće se liječi kirurškom internom stabilizacijom. S obzirom na učestalost ovog prijeloma, traga se za optimalnim načinom fiksacije koji se ispituje biomehaničkim numeričkim analizama. U ovom radu, uspoređena su dva različita načina opterećenja: opterećenje u karakterističnim točkama i reducirano opterećenje na vrhu glave femura. Za uspješno koštano cijeljenje potrebno je izvršiti repoziciju i osigurati mirovanje ulomaka što se u mnogim slučajevima ostvaruje fiksacijom kanuliranim vijcima u konfiguraciji obrnutog trokuta. Dosadašnji način opterećenja uzima u obzir samo silu u smjeru vertikalne (z) osi, odnosno opterećenje se zadaje kao umnožak 1/2 težine čovjeka i faktora sigurnosti 2 ili 3.

Abstract (english)

Femoral head fractures are most commonly treated with surgical internal stabilization. Given the frequency of this fracture, the optimal fixation method is being investigated by biomechanical numerical analysis. This paper compares two different loading types: loading at characteristic points and reduced loading located on the top of the femoral head. For successful bone healing, it is necessary to reposition fragments and ensure their position is fixed which is often achieved by fixation with cannulated screws in the 'inverted triangle' configuration. Current methods of loading consider only forces in the vertical (z-axis) direction, i.e., the load is given as a product of 1/2 human weight and safety factor of 2 or 3.
