Abstract

U radu je provedena analiza naprezanja, deformacija te su određeni najveći pomaci za jedan
generalni slučaj opterećenja dvaju različitih glenoidnih implantata. U teorijskom dijelu
prikazana je anatomija ramenog pojasa, kinematika i opseg pokreta, s posebnim naglaskom na
kosti kao strukturne dijelove ramena. Također je opisan i glenohumeralni zglob, kao
najznačajniji zglob u ramenu. Uz opis bitnih svojstava i značajki ramena vezanih za ovaj rad,
navedeni su najčešći uzroci boli u ramenu i načini njihova liječenja, kao i indikacije za kiruršku
ugradnju unutrašnjeg umjetnog zgloba - endoproteze. S obzirom da je između ostaloga zadatak
bio modelirati glenoidne implantate koji su sastavni dio postupka ugradnje totalne endoproteze
ramenog zgloba, poseban dio rada je posvećen opisu navedenog operativnog zahvata, te
vrstama i načinima fiksacija glenoidnih komponenti, te anatomskim parametrima ključnima za
pravilnu predoperativnu odluku. U drugom su poglavlju opisane i modelirane odabrane
glenoidne komponente kao zasebne konstrukcije. Definirane su im dimenzije s obzirom na
dostupne podatke na internetu, a nedostupni podaci za izradu modela određeni su proizvoljno s
obzirom na prirodu opterećenja. Prije početka numeričke analize modela implantata u
programskom paketu Abaqus, u trećem poglavlju dan je kratak prikaz načina pozadine rada i
teorijske podloge navedenog programskog paketa. Prilikom analize korišteni su osnovni
tetraedarski elementi drugog reda za slučaj djelovanja mehaničkog opterećenja. Neposredno
prije analize izvršena je verifikacija primijenjenih konačnih elemenata na poznatom
pojednostavljenom problemu čija rješenja je moguće dobiti analitičkim putem. U posljednjem
poglavlju provedena je numerička analiza za dva različita tipa modela glenoidnih implantata
prilikom djelovanja istog tipa mehaničkog opterećenja. Za svaki model prikazani su rezultati
raspodjele pomaka i naprezanja. Tako dobiveni rezultati uspoređeni su s dostupnim analitičkim
rezultatima i na temelju toga iznesen je zaključak.

Abstract (english)

In this paper was performed the stress analysis, the deformations were determined by the largest
displacements for one general case of loading of two different glenoid implants. The theoretical
part shows the anatomy of the shoulder girdle, kinematics and range of motion, with special
emphasis on the bones as structural parts of the shoulder. The glenohumeral joint has also been
described as the most important joint in the shoulder. In addition to the description of the
essential properties and characteristics of the shoulder related to this work, the most common
causes of shoulder pain and methods of their treatment are listed, as well as indications for
surgical implantation of an internal artificial joint - endoprosthesis. The task was to model
glenoid implants that are an integral part of the procedure of a total shoulder arthroplasty. A
special section is dedicated to the description of the operation, the types and the methods of
fixation of glenoid components and anatomical parameters crucial for proper preoperative
decision. In the second chapter, selected glenoid components are described and modeled as
separate constructions. Their dimensions were defined with respect to the data available on the
Internet. The unavailable data for modeling are determined arbitrarily with regard to the nature
of the load. Before the beginnig of the numerical analysis of the two different implant models
in the Abaqus software package, the third chapter gives a brief overview of the mode of
operation and the theoretical background of the software package. During the analysis, basic
tetrahedral elements of the second order were used in the case of mechanical load. Immediately
before the analysis, the verification of the applied finite elements on a known simplified
problem was performed, the solutions of which can be obtained analytically. In the last chapter,
a numerical analysis was performed for two different types of glenoid implant models under
the action of the same type of mechanical load. For each model, the results of the displacement
and stress distribution are presented. The results thus obtained were compared with the available
analytical results and a conclusion was drawn on that basis.
