Abstract

U ovom radu razmatrana je digitalizacija endoskopskog kirurškog zahvata otvaranja čeljusnog (maksilarnog) sinusa, obuhvaćajući posebno: snimanje, analizu, definiciju te digitalizaciju i virtualizaciju trajektorije otvaranja i njenog ostvarenja. Opisane su anatomske strukture povezane sa zahvatom, specifični instrumenti i uređaji koji se koriste u zahvatu, te sam postupak otvaranja maksilarnog sinusa. Trajektorije otvaranja maksilarnog sinusa digitalizirane su u softveru Slicer, pomoću Python skripte na temelju CT snimkama šestero bolesnika, te je tako dobivena uprosječena trajektorija otvaranja. Potom se u okružju operacijske sale u virtualnoj stvarnosti, korištenjem opreme HTC Vive 1, simuliralo pokrete kirurga i dobivena je trajektorija slična onoj uprosječenoj iz Slicera. Iako je u nekim svojim aspektima virtualizacija zahvata pojednostavnjena, jasno je pokazan potencijal virtualne stvarnosti kao alata za bolje razumijevanje i unaprjeđenje kirurškog procesa.

Abstract (english)

In this paper, the digitalization of the endoscopic surgical procedure for the opening of the maxillary sinus is discussed, specifically encompassing: imaging, analysis, definition, digitalization, and virtualization of the trajectory for opening and its execution. The maxillary sinus and associated anatomical structures are described, along with the specific instruments and devices used during the procedure, as well as the process of opening the maxillary sinus itself. The trajectories for opening the maxillary sinus were digitalized using Slicer software with a Python script based on CT scans of six patients, resulting in an averaged opening trajectory. Subsequently, in the operating room environment using virtual reality equipment (HTC Vive 1), surgeon′s movements were simulated, producing a trajectory similar to the averaged one from Slicer. Although some aspects of the virtualization were simplified, the work clearly demonstrates the potential of virtual reality as a tool for improving the understanding and enhancement of surgical processes.
